چکیده
A 55-year-old man with carcinoma sigmoid colon (fluorouracil [5-FU]: cumulative dose of 6,600 mg/m; and oxaliplatinum: 245 mg/m) presented with encephalopathy and pancerebellar involvement 2 weeks after receiving a third cycle of chemotherapy. Brain MRI showed diffusion restriction in bilateral deep white matter, cerebellar peduncles, and splenium of corpus callosum (figures 1 and 2) suggestive of 5-FU–induced leukoencephalopathy. Symptoms improved after discontinuation of chemotherapy as expected in drug-induced encephalopathy. Diagnosis of PRES (posterior reversible encephalopathy syndrome) seemed less likely because it usually involves posterior subcortical white matter. Diffusion-weighted MRI is a useful modality for early detection of this characteristic encephalopathy. AUTHOR CONTRIBUTIONS Sahil Mehta: data collection, writing of manuscript. Gagandeep Singh: revision of manuscript, concept of manuscript. Birinder Singh Paul: data collection, review of literature.

متن کامل5-FU induced acute toxic leukoencephalopathy: early recognition and reversibility on DWI-MRI.
Acute toxic leukoencephalopathy (ATL) is a rare adverse effect of 5-Fluorouracil (5-FU) chemotherapeutic agent. It is imperative for the radiologist to confidently identify the white matter changes caused by this agent in case of toxicity. This will help in early detection and appropriate management of patient, as the condition is reversible both clinically and on imaging. We report a case of a...

A boy was diagnosed at age 10 months with megalencephalic leukoencephalopathy with subcortical cysts (MLC) based on progressive macrocephaly and characteristic MRI findings (figure, A–C). Over subsequent years, initial motor delays resolved and neurobehavioral difficulties were mild. On repeat MRI at 6 years (figure, D–F), only small areas of frontal and temporal white matter signal alteration ...

This is a 57-year-old right-handed male with a history of locally advanced esophageal adenocarcinoma, who presented with confusion and speech changes. Patient received 5-Fluorouracil (5-FU) and cisplatin four days prior to presentation, which was complicated by very poor oral intake on day 2 of therapy. On the day of presentation, he was confused and his speech was slurred. The remainder of the...

At age 8 months, an infant girl displayed rapid developmental regression. Family history, birth, and initial development were unremarkable. After hospital admission, cerebral MRI showed bilateral cystic lesions in the centrum semiovale. Follow-up imaging after 3 months demonstrated a dramatic progression in these alterations with demyelination of the supratentorial white matter (figure). Bioche...
